What Information We Collect
We collect essential personal and health information from you through two forms:

Client Intake Form (completed before your first booking)

Pre-Massage Form (sent before each appointment)

Contact details (full name, phone number, email, home address)

Date of birth (for age verification and appropriate care)

Emergency contact details

Medical and health information (current and past conditions, medications, injuries, allergies, pregnancy status, etc.)

We also collect proof of identification (e.g. driving licence, passport) via email for age verification. For clients under 18, we request proof of guardianship (such as a birth certificate or adoption certificate) to ensure that the person booking is the child’s legal guardian. These are submitted securely via email as this cannot be done through our forms.

How Your Data Is Stored
All data is stored securely on a private, password-protected Google Drive, accessible only to authorised therapists at Fawx Massage Therapy.
How Long We Keep Your Data
Intake forms are kept for 10 years after your last appointment (or until your 25th birthday for under 18 clients, whichever is longer.)

Pre-massage forms are kept for 10 years after each appointment (or until your 25th birthday for under 18 clients, whichever is longer)
